Our commitment to your privacy

This HIPAA Notice of Privacy Practices explains how Joshua Farrow, BCBA/LBA may use and share your protected health information (PHI), as well as your rights and responsibilities regarding that information. We follow HIPAA regulations and applicable Virginia laws to keep your information confidential.

How we may use and disclose information

  • To provide, coordinate, or manage services and supports that you request.
  • To communicate with caregivers or other providers with your authorization or when permitted by law.
  • To comply with legal and regulatory requirements, including Medicaid waiver documentation.
  • In emergency situations when disclosure is necessary to prevent serious harm.

Your rights

  • Request access to or copies of your records.
  • Ask us to correct information you believe is inaccurate or incomplete.
  • Limit how we use or share your information, within the allowances of HIPAA.
  • Receive an accounting of certain disclosures of your information.
  • File a complaint if you believe your privacy rights have been violated.
